CHANGE OF AUTHORIZATION IN SUBSCRIPTIONS
The steps required to complete the change of authority are set out below.
You need to send the documents to the email@example.com address for the completion of the transactions. After the necessary examinations, Organik Haberlesme Customer Representative will call you and complete the transactions.
• Authorized Change Form
• Copy of Authorized ID
• E-government Settlement Certificate
• If the transaction is carried out as a proxy, Power of Attorney in addition to the documents
• Proof of authorization:
• For Limited and Joint Stock Companies: Photocopy of Signature Circular
• For Municipalities: Certificate of Authorization or Sealed Letter of Authorization Signed by the Mayor Stating that He is Authorized to Act on Behalf of the Institution
• For Associations, Foundations, Trade Unions: Photocopy of Authorization Certificate or Photocopy of Decision Book (First Page and Decision Page Certified by Notary)
• For Political Party: Certificate of Authorization or Sealed Letter of Authorization Signed by the Authorized Party, Stating that You Are Authorized to Act on Behalf of the Party
• For Government Agencies: Certificate of Authorization or Sealed Authorization Letter Signed by the Authorized Authority Stating that the Authority is Authorized to Act on Behalf of the Institution
• For Schools: Certificate of Authorization (Appointment Decision - Letter of Appointment) or Sealed Letter of Authorization Signed by the School Principal Stating that He is Authorized to Act on Behalf of the School
